Does that mean I need a knee replacement?”

This is one of the most common questions patients ask me. And my first answer is simple:

No. Not every patient with knee pain needs a knee replacement.

Knee replacement is a major procedure, and the decision should not be made simply because you have arthritis on an X-ray or because you have crossed a certain age.

The more important question is, if you are older (55 – 60 years +), how much is your knee affecting your everyday life? For younger, osteoarthritis is less likely.

Why does the knee start hurting?

One of the common causes of persistent knee pain, particularly as we get older, is osteoarthritis. For younger, osteoarthritis is less likely, and we should look for other treatable causes.

The cartilage that normally allows the bones in the knee to move smoothly can gradually wear down. As the condition progresses, patients may experience:

Pain while walking

Difficulty climbing or using stairs

Pain after sitting for a long time

Knee stiffness

Swelling

Difficulty fully bending or straightening the knee

Reduced walking capacity

Gradual deformity of the knee

But the severity and symptoms can be very different from one person to another.

When should you start considering knee replacement?

In the early and moderate stages of arthritis, many patients can get significant relief with non-surgical treatments.

Depending on the individual patient, this may include:

Physiotherapy

Appropriate medication

Weight management

Activity modification

Strengthening exercises

Other conservative treatments

However, when the disease progresses and these measures are no longer providing adequate relief, knee replacement may become an option.

Some signs that it may be time to discuss surgery include:

1. Pain is interfering with your daily activities

If walking, shopping, climbing stairs, going to work or doing normal household activities has become difficult because of your knee, it is time to get the problem evaluated.

2. You have significant pain even at rest

Pain that continues even when you are resting, particularly pain that affects your sleep, can indicate more advanced joint disease.

3. Your walking distance has reduced significantly

If you could previously walk comfortably but now have to stop after a short distance because of knee pain, this is an important sign.

4. Your knee has become very stiff or deformed

Difficulty fully straightening the knee, significant loss of movement or progressive bowing of the knee may indicate advanced arthritis.

5. Non-surgical treatment is no longer giving sufficient relief

If appropriate treatment has been tried but pain and disability continue to affect your life, surgery may need to be considered.

Does an X-ray alone decide whether I need a knee replacement?

No.

An X-ray is extremely useful because it helps us understand the condition of the joint. But treatment decisions should not be based on the X-ray alone.

Two patients can have similar-looking X-rays but completely different levels of pain and disability.

One person may have significant arthritis but still function reasonably well. Another may have similar changes but struggle to walk, climb stairs or sleep.

Therefore, we consider:

Your symptoms + physical examination + X-rays + activity level + overall health

before recommending a treatment.

Should I delay knee replacement for as long as possible?

This is another common misconception.

Some patients continue suffering for years because they believe:

“The longer I wait, the better it will be.”

But if your pain is continuously increasing, your mobility is reducing and your quality of life is deteriorating, there may be little benefit in postponing treatment simply because of fear.

At the same time, surgery should not be recommended just because arthritis is visible on an X-ray.

If you are above 55–60 years of age, have already tried appropriate treatments, and your knee pain continues to affect your activities of daily living (ADLs), it may be the right time to consult a knee replacement specialist and discuss whether knee replacement is appropriate for you.

The right time for knee replacement is different for every patient.

The goal is not to operate early or late.

The goal is to choose the right treatment at the right time for the right patient.

If knee pain is affecting your walking, sleep, stairs or everyday activities, get your knee properly evaluated. I’d keep it strong but medically responsible:

Delaying the decision for too long can sometimes make treatment more complex. A procedure that could have been relatively straightforward may become more challenging, recovery may take longer, and the overall cost may increase. In advanced cases, certain patients may also no longer be suitable candidates for routine knee replacement and may require a more specialised approach.

Not every knee pain needs a replacement. But when replacement is genuinely required, unnecessary delay may also not be the right approach.

If you would like to learn more about knee and hip conditions, treatment options and joint replacement, you can explore the Arthritis & Joint Replacement Clinic and https://share.google/0mV4NvUDl6XZHdfVu

“Doctor, my knees hurt a lot.
